FAQ about the form:

 

What do I enter on the "Payment Method" line?
You may pay online with Pay Pal or Credit/Debit Card OR you may pay in person with cash, check, or money order.  On the line that asks "Payment Method" please let us know which form of payment you will be using.

 

How do I pay online?

You may pay online with Pay Pal or credit/debit card by clicking on the link below.  There is a drop-down box in which you may select the class cost.  If you are paying for more than one child at different prices, you may "add to cart", then "continue shopping" to add the next child.  If you are paying for more than one child, but at the same price, when you "add to cart" you may change the quantity to the number of classes for which you are paying. 

 

How do I pay in person?

Cash and check payments must be made prior to the beginning of the first class.  Plan to arrive at least 10 minutes early to make your payment OR mail  your payment to Amelia Buechler - C/O WAUMC - 359 E Markison Ave - Columbus OH 43207.  If you send via mail, allow enough time for it to arrive before the first class.  Checks should be made payable to AMELIA BUECHLER.

 

I don't have 4 children; what do I put in the extra spaces?

There is room on this form for you to enroll up to 4 children in the same family.  If you are enrolling fewer children, leave the extra spaces blank. 

 

I have more than 4 children; how do I enroll them? 

If you need to enroll more than 4 children, please include the information for additional children in the "Anything else we should know?" field.

 

I want to enroll my child and a friend or relative; how do I do that?

Unless you are the legal guardian and the child lives with you, please fill out the form for your child, send the information, then return to our website and fill out the form again for the additional child, using their current contact information.  In the "Anything else we should know" box at the bottom of the form, you may make a note that your child and the other child are friends or relatives.

When you come to the first class, you will be given a printed copy of your enrollment form for each child.  Please confirm that all information on the printed form is correct, sign, and return to us.  The following disclaimer will be printed on the bottom of the enrollment form: 

I understand that Tippy Toes dance classes are non-professional classes for beginning dancers.  I also understand that my dancer is to follow class safety rules, including proper warm-up and spacing.  In the unlikely event of injury I accept responsibility for the care of my dancer. A $35.00 fee will be charged for returned checks.
